The staff at Princeton IVF welcomes new patients to our practice. We understand the stresses that come from fertility issues, and want to make your first visit with us as pleasant as possible.

In order to set up your initial consultation with the doctor, you will need to speak with Raquel our Infertility Benefits Coordinator.  Her phone number is 609-896-4984, option 2, her fax is 609-896-3266 and her email address is raquel@delvalobgyn.com. She will ask for your demographics and insurance information and set up your consultation with the doctor.

In order to get the most out of your consultation, we ask you to bring the following items to your visit:

• any records of fertility or hormonal related testing or treatments
• your spouse (if available)
• your insurance card and any referrals or copays required
• your completed face sheet and HIPAA forms if you are new to the practice
• the name and address of your ob/gyn and/or PCP

You should plan to arrive at least 15 minutes early to fill out paperwork. In order to expedite this process, you may go to our office forms and print and fill them our.  Click here to link to our forms page.

Please note that our office has a 24 hour cancellation policy for consultations.  If you are unable to make your appointment and do not notify us, you may be charged a cancellation fee.
